Application for Diplomate Examination

The complete application for examination to become a Diplomate of ICAK must be received by the Corresponding Secretary 60 days before the examination. Paper submissions, in English, with chapter certification. must be received at least 90 days in advance.

You can either download and fill in the form and post it back with the appropriate fee, or fill in the online form below.

• The number of Accredited Hours of A.K. study must be itemized and typed on a spreadsheet or Word document and sent with  the “Course Credit Report” or equivalent, showing signatures of the Certified Teaching Diplomate instructors currently certified by the International Education Board (IEC) at the time of the class.
• List the titles and dates of two papers accepted for publication in ICAK Collected Papers (any chapter) and evaluated by the Literature Review Committee of a Chapter on your application.
• Submit copies of papers to the examining board with completed Chapter Paper Review forms

“Refund Policy: If an applicant decides to withdraw from the examination process before beginning the examination, the examination fee will be refunded less a 40%  administrative fee. After the examination process has been started, no refunds will be granted.”
